Our Services

Our Services are bespoke and person-centred designed to deliver high quality standards of support and care to meet each service user’s needs. From assessment to recruiting staff, training staff and finally, delivering support or care in your own home.

Our Approach

Your Social Care LTD provides bespoke support /care services to adults with Learning Disabilities, Autism, ADHD and Mental Health within the Transforming Care Agenda, we provide Supported Living services based on two service models Enhanced/Intensive Support (principle 7 of the service model) and Community-based Forensic Support (principle 8 of the service model). We believe that people with disabilities and/or behaviors that may challenge are entitled to lives which are as full as anyone else’s. 

We believe in Right Support, Right Care and Right Culture. Co-production is at the centre of how we aim to deliver high quality standards of care and support, underpin by person-centred approach in everything we do.

Our aim is to offer high quality services, by respecting the wishes, feelings and preferences of the service user and uphold their human rights and dignity. Our priority is to ensure our services safeguard and promote the health, independence, welfare and quality of life for our service users whilst they continue to reside in their own homes in the community. Core aims;

  • To provide person-centred care/support with focus on outcomes for the service user, using person-centred tools and research to achieve this aim.
  • To support people to develop the knowledge, skills and confidence they need to effectively manage and make informed decisions about their own care and support by empowering them.
  •  To understand what behaviors that may challenge exhibited by the service user is trying to communicate to us, by creating a co-production (parents, family members, professionals and us) team around each service user to better understand and meet their needs.
  • To ensure the service is provided in a non-discriminatory way and respects each service users’ rights to independence, confidentiality, privacy, dignity, fulfilment, make informed choices wherever possible.
  • To ensure each service user’s protected characteristics and values are respected in relation to religion, culture, race, ethnic origin, sexuality, sexual orientation, political affiliation, disability or impairment.
  • To provide a high-quality workforce who are well-led, supported and well trained relevant to the services they’re delivering.
  • To Provide Service User specific training through a thorough assessment of the individual’s needs, then provide staff with training of each identified area of required further training, for example – Further training on understanding and how to prevent sexual exploitation, deeper understanding of ADHD, Autism etc. This would enable staff to have a deeper understanding on how to better support the service user with coping strategies, resilience etc so that they can focus on their aspirations and living a high quality of life.
  • Using Positive Behavior Support, Active Support, Clinical Identified Strategies and PROACT-SCIPr as support model and strategies.

We aim to integrate the people we support into the community by renting properties in the private renting market and thereby, strategically place them within the communities of their choosing in cooperation with relevant agencies, MAPPA, Social Services, MOJ where applicable.

Staffing

All our staff will be comprehensively trained to acquire necessary skills to support the service user they’re supporting or care for. No member of staff shall work with any of our service users without undergoing relevant training identified during the assessment of that person as necessary to support/care for the person they’re working with.
